Documents requested for all new students:
-NEW Tennessee Certificate of Immunization form ( # PH-4103 ). Families needing immunization and medical records help, contact the district liaison.
-Birth Certificate or proof of birth
-Social Security Card (Optional)
-Proof of City Address (for city students only) Families with no proof of address, contact the district liaison for assistance.

Requirements for new pre-k and kindergarten students:
-Pre-K: Children must be 4 years of age on or before August 15 for the upcoming school year.
-Kindergarten: Children entering kindergarten shall be five ( 5 ) years of
age on or before August 15 for the upcoming school year.
-Proof of Age: A Birth Certificate or hospital record
-Pre-K & Kindergarten: NEW Tennessee Certificate of Immunization form ( # PH-4103 )
-Health Examination: Physical to be filled out in the top right hand corner of form
-Social Security Card
-Proof of income (for Pre-K only) IE: 1st Page of Income Tax or Zero Income Document
-Proof of city address for Kindergarten and Pre-K: Parents must provide proof of residency such as a copy of a signed lease agreement or mortgage statement, utility bill (i.e. electric, water, gas, or sewer), bank or credit card statements, paystub.
When registering, if the electricity is not in your own name, you must submit a signed legal affidavit obtained at the school and present it with the electric bill. The person whose name is on the electric must be present.
Tuition for Non-Resident Students:
All city resident students will be placed. If classroom space and teacher capacity are available, we use a priority-tier system for non-resident applications to manage our growth while being fair to our families. We process these applications in order, ensuring every qualified student in a higher tier has a seat before moving to the next tier:
Tier 1: Children of Dayton City School faculty, staff members, and City of Dayton employees. (Exempt from tuition)
Tier 2: Returning non-resident students currently enrolled and in good standing.
Tier 3: Siblings of current non-resident students in good standing.
Tier 4: All other new non-resident applicants.
The School Board has established the following rates for students residing outside the city:
First Student: $500
Additional Sibling: $250
Family Maximum Cap: $1,500
Flexible Payment Options:
Per the revised policy, families may choose to pay in full at the start of the year OR in two equal installments due at the start of each semester. Please note that tuition fees are non-refundable, unless a student is not accepted due to a lack of space.
